After 10 years of operation the Clarence City Council is evaluating the Live Well, Live Long Program to assess its impact and help it be effective into the future.

As a Facilitator of the Live Well, Live Long Program, which now operates in several locations across Tasmania, we are keen to learn from your experience to help inform the evaluation of Clarence City Council's Program.
We would appreciate if you could take about 5 minutes to complete this survey. Its focus is mainly administrative but there might be some overlap with the topics covered during the online discussion with Andrea Young in July, 2023.
